Efficacy of Large Loop Excision of the Transformation Zone(LLETZ) in the Evaluation and Treatment of Cervical Neoplastic Lesions

Abstract
Large loop excision of the transformation zone(LLETZ) is a new diagnostic and therapeutic approach for management of cervical intraepithelial neoplasia. This study was performed to assess the efficacy of LLETZ in the diagnosis and treatment of
abnormal
cervical lesions. Two hundred six patients with abnormal cervical cytology were treated with LLETZ under local anesthesia as outpatients from July, 1993 to June, 1994. Histological comparison between the cervical cytology and LLETZ specimens
showed
63.2%(130/206) of concordance rate. Histological comparison between the colposcopic biopsies and LLETZ specimens showed agreement with only 69.4%(75/108) of case illustrating identical degrees and 79.2%(84/108) within one degree difference. The
margin
involvement of LLETZ was seen in 37.5%(51/136) but the residual lesion after hysterectomy was only seen in 34.8%(16/46). Severe delayed vaginal bleeding was seen in 1.5%(3/28) but there was no other serious complication. LLETZ was used as the
conservative treatment method in 59 patients with CIN, and the success rate is 91.5%. In conclusion, LLETZ is an effective, quick and reliable procedure on accurate diagnosis of cervical pathology and conservative treatment of cervical
intraepithelial
neoplasia.
